Forms & Requirements
1. Athletics Registration: In order to try out for a John Carroll team you must register using Magnus Health. Please note, the School is transitioning away from SchoolDoc and moving to a new system, Magnus Health. To register for tryouts, access your Magnus Health SMR account through Veracross by clicking on the Magnus button in the Parent Portal, or use this link and log in with your Veracross credentials.
2. Physical Forms: Physical forms must be uploaded to Magnus Health. The physical examination must be completed and signed by a physician, physician’s assistant or nurse practitioner. The Health Questionnaire must be completed and signed by the student-athlete and parent/guardian, and reviewed by the physician, physician’s assistant or nurse practitioner. Also the student-athlete and parent must also sign and date the consent, release, and assumption of risk form. A physical is valid for 365 days, as dated by the medical provider. We prefer all student-athletes be on a June/July to June/July schedule, so as the physical remains valid for the duration of the school year. Special circumstances do arise. Please contact the Sports Medicine Staff with specific questions. It is preferred that all physicals are turned in each school year by August 1.
3. Neurocognitive Baseline Testing (ImPACT): This is a computer-based testing tool used in the management of mild traumatic brain injuries (mTBI), more commonly known as concussions. The student-athlete will take a baseline test, so that in the event you sustain a concussion or an injury with a mechanism that suggests a concussion, we may be able to evaluate and access the severity of the injury and the progress of your recovery. This is mandatory for the freshman and junior class student-athletes and any student new to athletics or transfer students. Testing is available for all student-athletes, however not required for boys or girls cross country, boys and girls tennis, boys or girls swimming, boys or girls indoor track and field, boys or girls outdoor track and field, boys tennis, and golf.
